Position Summary
The ideal Case Manager & Housing Navigator builds and fosters healthy relationships with TAY and assists with helping them secure permanent housing; Provides life coaching and case management services to address multiple complex needs and reduce barriers, including referrals to UBH programs and/or other community resources. This position will work in our Transitional Housing Program for Transition Age Youth (TAY) families located in South Los Angeles.
ESSENTIAL RESPONSIBILITIES:

Provide life coaching and support services applying strengths-based, Positive Youth Development, and participant-centered -
Develop a service and housing plan to establish goals and action steps toward stabilization, independent living, and self-sufficiency in conjunction with the -
Help youth develop permanent connections in their lives to strengthen their support -
Assist youth in identifying and accessing services necessary to reach long-term housing -
Utilize personal vehicle, in good working order, to transport youth to appointments when -
Meet with youth weekly, to monitor progress and /or barriers toward achieving their life aspiration -
Complete clinical services assessment for each youth and share results with LCSW for next steps. -
Provide crisis intervention services as -
Work with TAY Education and Vocational Specialist to support youth in their education and employment -
Work with UBH team members and community partners, and with input from youth, develop and implement engaging activities and -
Provide and/or coordinate life skills such as parent-child communication, healthy development, healthy life skills (communication, sexual health and development etc.), relationship skills, career success, financial -
Help youth identify and find housing options including assisting with completing housing and credit check -
Perform recordkeeping and reporting of program activities on electronic reporting systems such as HMIS for outcomes and evaluation; and complete progress notes and program-related reporting as -
Report issues and problems to supervisor and -
Attend staff meetings. -

QUALIFICATIONS:

Bachelors degree from an accredited college or university in social work, human services or a related field required; Masters Degree in Social Work preferred. -
Minimum of two-years experience providing services to persons experiencing homelessness, preferably Transition Age -
Demonstrated experience in working with at-risk youth to reach their housing, employment, or education -
Demonstrated experience in one or more of the following areas: mental health, chemical dependency, emotional/behavioral special education, at-risk youth -
Demonstrated knowledge of and ability to work effectively with youth with a variety of identities, cultures, and backgrounds including ethnic/racial minorities and LGBTQ -
Knowledge of Trauma-Informed Care and Practices a -
Ability to work collaboratively, but -
Strong written and verbal communication -
Creative problem-solving -
Ability to work in a -
Exceptional organizational -
Bilingual (English/Spanish) preferred. -
Ability to work with diverse population. -
Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook). -
Experience using a data management system to record client information and
MANDATORY REQUIREMENTS:

Valid California Drivers License and proof of auto -
TB Test Clearance (upon hire). -
Successful completion of background screening.
