We are looking for a full-time Nurse Practitioner to join our Jersey Shore Skilled Nursing and Rehabilitation Center in Jersey Shore, PA. Our medical group is committed to enhancing health outcomes for residents in skilled nursing and senior living facilities. Our team, which includes physicians, nurse practitioners (NPs), and physician assistants (PAs), is backed by a clinical leadership team with extensive experience in post-acute and long-term care. Position Responsibilities- The Advanced Practice Provider will report to the Director of Clinical Practice and/or the Area Clinical Practice Manager.
- Collaboration with the Center Leadership Team will be essential to meet center-specific goals and priorities.
- Work alongside the Medical Director and/or Attending Physician to deliver patient-centered medical care.
- Provide primary and urgent care services to residents, working in conjunction with other medical professionals at the center.
- Contribute to our mission of enhancing lives through high-quality healthcare and compassionate service.
- Must be a graduate of an accredited School of Nursing and hold a valid registered nurse license.
- Certification as a Nurse Practitioner is required, with a Master's Degree in Nursing focusing on Older Adult and/or Family Nurse Practitioner (FNP) specialties.
- A minimum of two years of experience working with adult or geriatric populations is preferred.
- Must possess an unrestricted professional license and DEA for each state of practice.
