Job Opportunity: Licensed Practical Nurse

Our organization is in search of a dedicated and empathetic Licensed Practical Nurse (LPN) to enhance patient-centered care within our network across Connecticut. This role is essential in delivering direct bedside assistance and ensuring the well-being of our patients.

Key Responsibilities
  • Provide direct patient care, including administering medications and treatments.
  • Monitor and record vital signs accurately.
  • Maintain comprehensive documentation in electronic health record (EHR) systems.
  • Collaborate with Registered Nurses (RNs), physicians, and care managers to implement effective care plans aimed at improving health outcomes in the community.
  • Educate patients and their families about health management and care processes.
  • Facilitate smooth transitions of care and promote safety and infection control measures.
Qualifications and Requirements
  • Valid LPN license in the state of Connecticut.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively within a healthcare team.
  • Commitment to providing high-quality, accessible care to all patients.
