Wallace is seeking a dedicated Nurse Practitioner to serve as an empaneled primary care provider for new and established patients, delivering comprehensive, relationship-based care in a collaborative, team-based clinic setting.

Key Details:

  • Title: Primary Care Provider - Nurse Practitioner.
  • Compensation: competitive salary $140,000-$170,000 DOE, language differential available.
  • Schedule: Full time, 4 days a week 10 hour days
  • Location: Our Rockwood Clinic, 18633 SE Stark Street, Portland, OR

What You’ll Do:

  • Serve as an empaneled primary care provider for new and established Wallace patients across the lifespan.
  • Provide preventive care, health screenings, chronic disease management, and patient education to support whole-person wellness.
  • Perform clinical evaluations, physical exams, and histories.
  • Order and interpret diagnostic tests and labs.
  • Collaborate with the Care Team to develop longitudinal care plans and coordinate referrals, follow-up, and preventive services across the care continuum.
  • Partner closely with RNs, MAs, behavioral health, care coordination, and other interdisciplinary team members to deliver integrated care.
  • Maintain high quality documentation, follow up planning, and patient education.
  • Maintain a patient-centered approach that advances access, continuity, and quality outcomes for the communities we serve.

What We’re Looking For: This role is ideal for an NP with primary care experience who values relationship-centered, team-based care and is committed to serving diverse communities with compassion and clinical excellence.

Requirements:

  • Current, unrestricted NP license in Oregon
  • Current, unrestricted  DEA license
  • Current BLS certificate
  • Experience managing chronic conditions, preventive care, and complex primary care needs across diverse patient populations
  • Commitment to team based, patient centered care

Preferred:

  • Experience in community health, family practice, FQHC, or other outpatient primary care settings.
  • Experience supporting Medicaid or higher acuity primary care populations
  • Bilingual in Spanish (preferred, but not required)
  • Collaborative, team oriented, and grounded in patient centered values
  • Able to balance efficiency with thoughtful follow-up and sustained patient relationship-building.
