Position Summary:

The Population Health Manager supports the mission of FHCW developing, implementing, and overseeing initiatives that advance health equity, clinical quality, and outcomes for underserved patients. This role includes significant responsibility for grant-funded programs including Massachusetts DPH grants such as Massachusetts Breast and Cervical Cancer Program (MBCCP) —ensuring compliance, performance monitoring, reporting, and partnership with internal and external stakeholders.

Program Specific Essential Duties and Responsibilities:

• Lead development, execution, and evaluation of population health initiatives aligned with HRSA requirements and FHCW goals.

• Integrate population-level strategies into care teams, including prevention, chronic disease management, and social determinants of health.

• Use UDS, PRAPARE, EHR reports, and risk stratification tools to identify disparities and track performance.

• Supervise CHWs, coordinators, or other program staff; ensure workflow development, coverage, and training.

• Manage all Grant funded program activities such as MBCCP, SRH, including implementation, deliverables, reporting, and compliance.

• Serve as the primary liaison with program officers, ensuring timely communication and accurate reporting.

• Maintain documentation, meet deadlines, and ensure adherence to grant budgets in collaboration with Finance.

• Prepare progress reports, renewal submissions, and grant updates.

• Build relationships with hospitals, public health agencies, community organizations, and local coalitions.

• Represent FHCW in Grant Funder workgroups and community health initiatives.

• Participates in the development of new services, care delivery models, and digital applications.

• Creates a supportive, team-based environment that facilitates group decision making and employee empowerment


Supervisory Responsibility – Community Health Workers, Coordinators & other Program staff


Qualifications and Education Requirements:

One of the following combinations of education and employment experience must be met:

  • Bachelor’s degree in Public Health, Health Administration, Nursing, Social Work or related field.
  • 2+ years of experience managing complex grant-funded programs, preferred.
  • Strong written and verbal communication skills, excellent computer skills (Microsoft Word, Excel, and Outlook).
